    Kidney function refers to the ability of the kidneys to filter the blood. For this purpose, the amount of waste products in the blood is measured. When kidneys are not working sufficiently, the substance creatinine enters the blood. This is a waste product from the muscles. The more creatinine in the blood, the worse the kidneys filter. This also depends on age and gender.

    The MDRD formula measures the level of Creatine in the blood and expresses it in a number. This number is called the GFR (the glomerular filtration rate) or how well the kidneys filter. The report will say GFR (MDRD) is ...ml/min/1.73m2.

    The calculation depends on:

    • Age
    • Gender
    • Ethnicity


    Serum creatinine provides only limited information about glomerular filtration rate (GFR). Therefore, creatinine clearance is often estimated using Cockroft and Gault's formula. This formula uses age, sex, weight and serum creatinine.

    The recently developed 4-point MDRD formula estimates the true GFR. The formula, which is based on data from the Modification of Diet in Renal Disease Study, also uses age, sex and serum creatinine, but not weight. The Kidney Foundation in the Netherlands, England and America recommend the MDRD formula for early detection of kidney function disorders.

    The formula is as follows: GFR (MDRD) (ml/min/1.73 m2) = 186 X ( serum creatinine (umol/l) / 88.4)-1.154 X age (in years) - 0.203 X 0.742 (if female).

    If you have a different ethnicity, multiply your own estimated GFR by 1.21.

    At a GFR <60[1] ml/min/1.73m2, renal function is impaired and at a GFR <30 ml/min/1.73m2, renal function is severely impaired. The MDRD study is based on a study population of white and Negroid adults aged 18 years and older with chronic renal failure. As a result, some limitations apply to the use of this formula.

    Who does this formula not apply to?

    The MDRD formula is not calibrated for:

    • people under 18 years of age
    • people with a highly abnormal body surface
    • ethnic groups other than white or negroid
    • patients with muscular diseases, paraplegia or quadriplegia
    • patients with acute renal insufficiency
    • malnourished people
    • people with a vegetarian diet
    • pregnant women

    As of January 1, 2022, we use the new formula:

    You estimate the creatinine clearance with mathematical formulas. The most commonly used formulas are the Modification of Diet in Renal Disease (MDRD) formula and the Cockcroft-Gault formula. A new formula is the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI). These formulas are suitable for use in adults.

    The new CKD-EPI formula, like the MDRD formula, provides an estimate of glomerular filtration rate (GFR) based on serum creatinine concentration, sex, age, and race. The CKD-EPI formula corrects to 1.73 m2 of body surface area and displays the estimated GFR in ml/min/1.73 m2. The CKD-EPI formula is as reliable as the MDRD formula, but is more accurate in patients with relatively good renal function. At GFR 60-90 ml/min, the results of the CKD-EPI formula are closer to the true GFR than those of the MDRD formula.
